An initial Individual Services Plan (ISP) will be developed upon intake and updated at least annually thereafter, before, or during the individual’s birth month.
The ISP will reflect the individuals’
- informed vocational/employment choices
- how employment will be secured
- benefits planning
- consumer’s role and responsibilities in achieving the agreed-upon objectives and goals
- Funding of services and supports will also be addressed in the plan.
When appropriate, the roles and responsibilities of the individual’s parents, care provider, and/or any other significant stakeholders will be considered.
All efforts to achieve the objectives and goals will be discussed, and modifications will be made
Once the individual has secure employment, all services will be monitored for both their and their employer’s satisfaction. The plan will be reviewed frequently with the individual and funding source, and progress achieved toward each ISP objective will be documented.
